A1: Thank you for taking my courses. Please send me the following items:

A formal email mentioning the purpose of the recommendation letter (e.g., graduate school application, job application)
Your updated resume/CV
Your most recent academic transcript
An explanation of how my courses have contributed to your learning and academic/professional growth
Any specific points you would like me to highlight in the letter

Action items:

Allow me sufficint time to respond (e.g., 2-3 days) and to write the recommendation letter (e.g., 2-3 weeks).
Please send me a reminder one week before the deadline.
Please drop me a line if your application is successful. I will celebrate your success!

Q2: I am looking for research opportunities in your lab. How can I apply?

A2: Thank you for your interest in our research. Please find the action items below:

Visit my research lab and publications to learn about our research areas.
Check if we have any open research positions.
Prepare a brief research proposal (1-2 pages) outlining your research interests, objectives, and how they align with our lab's focus.
Send me an email with your research proposal, resume/CV, academic transcript, and sample publications (if any).
Be patient while I review your application. I will get back to you if your experience and interests are a good fit.
